Schwannoma WebPathology

Schwannoma is a slow-growing, encapsulated benign peripheral nerve sheath tumor that is usually seen sporadically (90% of cases). The remainder are associated with Neurofibromatosis 2 (3%), schwannomatosis (2%), or multiple meningiomas with or without NF2 (5%).Schwannomas may rarely arise in the setting of NF1. The most common locations are head, neck, upper and lower extremities, posterior. Schwannomas usually consist of a well-circumscribed encapsulated nodule. The fibrous capsule is derived from epineurium and includes the residual nerve fibers. Rare schwannomas arising in the dermis may be unencapsulated. Plexiform schwannomas are multinodular tumors, like plexiform neurofibromas Schwannoma is predominantly composed of Schwann cells which are consistently strongly positive for S-100 protein. SOX10 (nuclear stain) is another useful marker for Schwannomas . These two markers can be extremely helpful in making the correct diagnosis in schwannomas with extensive degenerative changes (Ancient Schwannomas) where the neoplastic cells are sparse and may be obscured by myxoid change or hyalinization

The nuclei are hyperchromatic and some show prominent nucleoli. The nuclear detail may be obscured by heavy melanin pigmentation which can be coarsely clumped or finely granular Melanotic (pigmented) schwannoma: Contains pigmented cells with melanosomes that express melanocytic markers. Usually arises around the spinal nerve roots. May show sheets of adipose-like cells and psammoma bodies. Large, vesicular nuclei with macronucleoli, high mitotic activity and necrosis portend worse prognosis

Often has a grape-like (botryoid) growth pattern Pilocytic astrocytoma - grade I is a circumscribed, well differentiated astrocytic neoplasm with piloid (hair-like) processes, most commonly occurring in children and young adults
